Case Manager, Housing Support, Mid West
Location: Clare
Contract: Full time, 1 year Fixed Term
The purpose of this role is to work as part of the Housing First Case Management Service, delivering a Housing First approach to support individuals experiencing long-term homelessness in County Clare.
What you will do:
Work in line with the principles and fidelity of Housing First and the objectives of the Housing First Intensive Case Management Service.
Operate within the policies and procedures of the Clare Housing First team and adhere to best practice standards.
Provide a safe, welcoming environment for clients who are out of home or at risk, respecting consumer choice and self-determination.
Build trusting relationships with clients, including those living on the streets or in temporary accommodation, and support them toward a stable lifestyle.
Conduct assessments and develop support plans using Care and Case Management systems, STAR Outcomes, and other tools to assist clients toward independent living.
Support clients with housing needs, welfare rights, entitlements, and access to move-on accommodation; assist in sustaining tenancies.
Maintain a portfolio of accommodation providers across social housing, homeless services, and private rental sectors.
Accompany and support clients at appointments (e.g., medical, legal, counseling) and help integrate them into their local community.
Promote life skills development, education, training, and employment opportunities as desired by clients.
Keep accurate casework records, daily logs, and assessments; input data into Focus Ireland systems and PASS.
Collaborate with multi-disciplinary teams, statutory and voluntary agencies; attend meetings, handovers, and training as required.
Ensure compliance with health, safety, and welfare standards; implement policies and contribute to service development.
What you will need:
A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology etc and/or be a CORU registered Social Care Worker
Experience of working with customers are or have been homeless
Experience of working with customers identified as having a dual diagnosis
Experience of teamworking
Report writing abilities
Experience of working and liaising with other agencies
Full driver’s license, with access to a car
Communication skills
Knowledge and understanding of homelessness and issues that affect people who are or have been homeless
Ability to plan and implement programmes
