This role is ideal for an experienced nurse who enjoys working with people, building relationships, and helping others deliver excellent care. Whether you already hold a management position or have experience coordinating care and supporting colleagues, we'd love to hear from you. As Nurse Care Manager, you will work closely with clients, families, healthcare professionals, and care staff to ensure every client receives safe, high-quality, person-centred care.
What You'll Do
Supporting Our Clients
Complete client assessments and develop personalised care plans.
Review and update care plans to ensure evolving needs are met.
Conduct regular client visits and quality assurance reviews.
Ensure care is delivered safely, effectively, and with compassion.
Support clients and families through changes in care needs.
Supporting Our Team
Provide clinical guidance and day-to-day support to care staff.
Help onboard, train, and develop new and existing team members.
Share best practices and promote continuous learning.
Foster a positive, respectful, and supportive team culture.
Working with Healthcare Partners
Liaise with GPs, Public Health Nurses, hospitals, and allied healthcare professionals.
Build strong relationships with clients, families, and community services.
Ensure effective communication to support the best possible client outcomes.
Quality & Compliance
Help ensure services are delivered in line with HIQA standards, HSE guidelines, and company policies.
Maintain accurate clinical records and documentation.
Participate in incident reviews, audits, and quality improvement initiatives.
Promote a culture of safety, dignity, and respect.
About You
You are someone who:
Is a Registered General Nurse (RGN) with active NMBI registration.
Minimum 3 years nursing experience in community care, homecare, older person services, disability services, or a related setting.
Enjoys supporting and developing others.
Builds trusting relationships with clients, families, and colleagues.
Communicates clearly and compassionately.
Is organised and comfortable managing competing priorities.
Has good IT skills and maintains accurate records.
Holds a full driving licence and is willing to travel within the Cork region.
