PERSON CENTRED WING

TEAM LEADER, SUPPORT SERVICES

Office location, Tullamore, Co. Offaly

The focus of this role will be to support an individual to realise their vision to leading a good life through Person Centred approaches in line with personal interests, skills, will and preference.

Permanent (Various Contracts)

We are seeking to recruit motivated, enthusiastic, creative and energetic Team Leaders, Support Services to support individuals with an intellectual disability to realise their vision for a ‘good life’.

The focus of the role will be to lead initiatives in supporting the individuals to live a life of their choosing and what matters most to them. The role will support the individuals to become connected and valued within their local community, to provide opportunities for new experiences and to assist each person in developing real and authentic relationships. The ideal candidate will have a proven track record in supporting individuals to develop valued social roles and also have experience in supporting individuals with communication needs and those who may display behaviours of concern and will have experience of person centred planning and knowledge of person directed and individualised support approaches.

Essential requirements/qualities of applicant are:

· Relevant Level 5 Qualification i.e. Healthcare Support, Health Service Skills, Applied Social Studies, Community Care, Early Childcare & Education, Special Needs Assisting or willingness to complete a NFQ Level 7 in health and social care or equivalent.

· Experience in delivering effective support services to vulnerable individuals.

· A recognised management qualification (Desirable).

· A valid licence to drive a manual vehicle in Ireland.

· Experience of developing and monitoring care plans, risk assessments, person centred plans and restrictive practices.

· Ability to form positive working relationships with people supported, families and professionals.

· Ability to adapt to the changing needs of people supported and the service.

· Professional knowledge and skills to undertake role.

· Experience of supporting adults with intellectual disabilities and/or autism.

· Experience of Person Centered Planning and New Directions.

· Excellent communication, organisational, logistical and I.T skills.

· knowledge and understanding of HIQA standards.

· Ability to maintain up to date and accurate records to ensure a high standard of care and support is always provided to each resident.

· Ability to work on own initiative as well as being a team player.

· Support, supervise and motivate the staff teams that report to them.

· Suitability as outlined in the introduction to the role.

Please note that the successful candidates will be required to be available for any day and sleepover shifts. In addition, we require staff members to be willing to work alone in the residents’ homes.
