Remuneration
The salary scale for the post is (01/06/2026):
€58,477, €59,871, €61,573, €64,767, €66,677, €69,056, €71,441, LSIs
New appointees to any grade start at the minimum point of the scale. Incremental credit will be applied for recognised relevant service in Ireland and abroad (Department of Health Circular 2/2011). Incremental credit is normally granted on appointment, in respect of previous experience in the Civil Service, Local Authorities, Health Service and other Public Service Bodies and Statutory Agencies.
As per HR Circular 012/25 Please note that previous experience working in the public service counts only where the individual was employed directly by the relevant Civil Service/Public Body. Details of service
The Integrated Care Programme for Chronic Disease (ICPCD) is a national programme focused on improving outcomes for people living with chronic diseases through the delivery of integrated, person-centred care across primary care, community services, acute hospitals and voluntary sector partners. The Programme supports the implementation of enhanced models of care for chronic disease management, including respiratory disease, diabetes, cardiovascular disease conditions. It aims to improve access, quality, coordination and continuity of care closer to home. The Grade VI Project Lead will support the delivery and implementation of national and regional priorities across the Integrated Care Chronic Disease programme in West Galway and City Hub, working collaboratively to advance integrated care pathways and service improvement initiatives.
Purpose of the post
The Grade VI Project Lead will support the planning, implementation, coordination, monitoring and evaluation of Hub initiatives within West Galway & City Integrated Care Programme for Chronic Disease Hub. The post holder will support project management, operational, analytical and administrative leadership to support the achievement of Hub programme objectives, ensuring alignment with national policy, HSE strategy and agreed service plans. The role will focus on supporting Integrated Care service delivery, on facilitating stakeholder engagement, on monitoring programme performance, on supporting governance arrangements and on contributing to service improvement initiatives that enhance outcomes for people living with chronic disease.
Post specific requirements
Candidates must demonstrate:
• Experience of working within a multidisciplinary and multi-stakeholder environment in support of service goals and objectives.
• Ability to manage competing priorities while delivering agreed objectives within defined timeframes.
• Strong organisational, analytical and reporting skills.
• Experience in preparing reports, presentations, briefing papers and performance information for senior management.
• Proficiency in Microsoft Office applications including Excel, Word, PowerPoint, Outlook and Teams.
• Knowledge of healthcare services and current health policy relating to integrated care and chronic disease management.
• Experience in stakeholder engagement and relationship management.
• Experience supporting change management and quality improvement initiatives.
• Knowledge of Sláintecare and Integrated Care Programme principles and priorities.
